Paper Copy
Patients may request a paper copy of their records by filling out a Medical Record Release Form. Please be sure to sign and date the document and submit the completed form to our Medical Records Department.

Electronic Copy
Patients may request an electronic copy of their records by filling out an online Medical Record Release Form or submitting a request via our patient portal to receive a mailed paper copy of your records. We may call you to answer some security questions to verify your identity.

Patients have the right to request that we deliver health information to you in a certain way, including secure email. You may request an electronic copy of your records by submitting a request via our patient portal or calling our office at 505-999-1600. You will need to answer some security questions to verify your identity, and then we will securely email your records.

WARNING: Transmitting patient information via email poses several risks, of which the patient should be aware. The patient should not agree to communicate via email without understanding and accepting the risk outlined in our email communication policy.
